PEPTIDE CARE CARDS

Question cards / Ready for the conversation

Bring the questions. Keep the answers.

Start with what you want help with. Use this list to keep the visit focused, then add the questions that matter to you.

  • Describe the problem before naming a treatment.

What I want help with is… What would you need to know before discussing treatment? What other options could fit?

Bring your medicine and supplement list, any relevant test results, and the questions left over from earlier care.

  • Ask what is being proposed.

What is the exact medicine name? Does it come as a shot, pill or something else? What problem would it treat? Is it a factory-made product or made to order? What studies support that use?

If a term is unclear, ask for it in ordinary words.

  • Ask who is responsible for each part.

Who examines me: a doctor, a nurse or someone else? Where can I check their Georgia licence? Which pharmacy supplies the medicine? Who explains the written instructions?

For an online visit, ask whether an exam in person is needed and how it would be arranged.

  • Ask about follow-up and the full cost.

Where can I send questions once the visit is over? How would we handle a treatment that does not fit? Which changes need urgent care?

What would I pay now and later? Are the medicine, tests and follow-up included? What are the cancellation terms?

  • Leave with answers you can find again.

Ask for the plan, the product information and the cost terms in writing. Note anything you still want explained.

There is room to ask another question before making a decision.
